U.S. senior military commanders ignored warnings that intelligence on potential Iranian targets was seriously outdated before approving strikes that included an attack on an Iranian girls’ elementary school that killed nearly 200 children and adults, CNN reported.

According to Jin10, CNN cited three people familiar with the decision-making process as saying the warnings were embedded in a key database and indicated the intelligence was based on data from years earlier and needed to be reviewed again.

The sources said adding targets to the strike list required approval by senior officers. Two of the sources said senior commanders disregarded the warnings for “efficiency” reasons because there was an urgent need to identify targets early in the war, and that this directly led to the mistaken bombing of the school.

The report said the strike was among the U.S. military operations in recent years that caused some of the most severe civilian casualties.
